Part 2 “Post-PT for ACL Reconstruction”

Functional Integration
Functional integration may begin with basic activity as early as 12 weeks post-surgery and can be progressed toward athletic activity between four and six months. The goal during this time (i.e., 12 weeks to six months) is to safely load the knee in all planes of motion without compromising the graft site.

Flexibility
Stretching the muscles around the knee is a priority for the client. Specifically targeting the quadriceps, hamstrings, and calves should maintain adequate flexibility around the knee.

Hamstring stretch
Ankle mobility
Groin/Hip/Thoracic
Calf stretch

Balance
Balance activity should have been implemented in
the early stages of rehabilitation. At the time of fitness activity, the client should have good balance with basic single-leg activities. Progressively challenging the knee in multiple planes will help prepare the joint for higher- level activity.

Strength training
Training should focus on developing symmetrical strength between the quadriceps, hamstrings, and hip muscles. Strengthening of the hip muscles helps restore stability in the lower limbs and decreases strain on the reconstructed ligament. The hamstrings and quadriceps are particularly important because they add stability to the injured knee and are critical in long-term knee function and prevention of further injury.

Month 6 – 9

  • Sport specific training and preparation to play phase.
  • Improve fitness, Improve strength, Improve proprioception, and Return to sports training.
  • Plan to return to sport when confidence has returned – usually 9 months

Month 9 – 15

  • Return to sport phase
  • At 9 months the knee will start to free up and feel less tight
  • Full proprioception to where the knee feels normal usually takes about 15 months.

“Most common causes for ACL tears”

Part 1 of a three part series on ACL tears…

ACL injuries are the most common sports-related injury of the knee. Seventy to 80% of ACL injuries are non-contact, with only 20 to 30% resulting from direct contact.

Pivoting and cutting sports such as soccer and basketball are the most common scenarios, whereas most direct contact ACL injuries occur in American football. Injuries often occur in relatively young athletic individuals 15 to 45 years of age. Females are two to eight times more likely to injure their ACL than males. Also, female basketball players are 7.8 times more likely to injure their ACL than male basketball players.

Mechanism of injury
The mechanism of injury often involves a maneuver of deceleration combined with twisting, pivoting, or side-stepping.

Signs/Symptoms
Most often a client will report hearing or feeling a “pop” during the activity, followed by immediate swelling, insta- bility, decreased ROM, and tenderness often occurs at the lateral line. This typically requires immediate medical care to immobilize and protect the joint, followed by a visit to the orthopedic doctor for further diagnosis and intervention.

Early Intervention
Postsurgery clients typically will be in physical therapy for the first three to four months, depending on the physician’s preferences and the client’s insurance constraints. The client is generally able to perform full weight bearing with crutches and a brace for the first two to six weeks. Physical therapy and a strengthening program continue until the injured knee has 90% of the strength of the other knee.

Common among protocols is a return to functional activity between 12 and 16 weeks after surgery and a return to sporting activities around six months. Clinical results of the ACL reconstruction are quite good, with very low reinjury rates.

The most common complication is residual anterior knee pain.

Who is at the “Greatest Risk for Hamstring Strain”?

The hamstring muscle group includes three distinct muscles: the biceps femoris, semitendinosus, and the semimembranosus. hamstring-muscles500

Of the three muscles in the hamstring the biceps femoris is the most likely to be injured. The straining or tearing of the long head of the biceps femoris muscle occurs particularly in athletes that perform quick acceleration or cutting in sports like football, soccer, and tennis.

Some factors that can predispose an athlete to hamstring strains are insufficient warm-up, fatigue, poor muscular coordination, excessive pelvis tilt, prior hamstring injury and imbalance in muscle strength between hamstring and quadriceps.

The athlete often becomes aware of the injury hearing or feeling a pop in the area of the hamstrings followed by immediate pain in the posterior.

Diagnosis of a hamstring injury is made primarily by obtaining details of the cause of the injury from the athlete and by performing a physical examination.

Recovery can occur as quickly as one week to six weeks or more depending on the grade of the strain. banner2-3

During the first phase of treatment hamstring strains follow the guidelines most soft tissue injuries follow; Protection, Rest, Ice, Compression, Elevation

About 7-8 days after the injury the second phase begins. Most experts agree that electrical stimulation, passive range of motion, myofascial release, and isometric exercises can be introduced.

The third phase occurs anywhere from one to six weeks after initial injury. Some of the stretching and strengthening exercises should include hip rotation. This is important because many of the sports require rotation, cutting, and change of direction.

The final phase of treatment is returning the athlete back to play. This includes sport specific movements, increasing hamstring strength and flexibility.

To prevent recurrence of the injury, the athlete should continue regular stretching and strengthening and should always warm up properly. Hamstring and hip stretching and continued strengthening of the quadriceps and hamstrings will help prevent re-injury.

One of the greatest risk for hamstring strain is prior hamstring strain, so complete and continued rehabilitation is necessary.

Iliopsoas Tendinitis

Iliopsaos tendinitis is most common in soccer players and runners who tend to perform repetitive hip flexion movements.

The most common symptom is pain down the front of the, sometimes radiating down the thigh. Activities such as kicking, running uphill, or hip flexion can exacerbate the symptoms. IMG_6568

Initial treatment includes ice, stretching, and strengthening of the area can be helpful.

Return to play usually happens within three to six weeks and should be gradual.

LCL (Lateral Collateral Ligament) Tear

Injury to the Lateral Collateral Ligament usually results from a direct blow to the inner knee toward the outside  or during a sharp pivot. With an injury to the LCL, there is local discomfort along the lateral knee.

In post-physical therapy the athlete will work on quadriceps-strengthening and balancing exercises to help reduce stress and strain on the knee.

Recovery can take up to three months. Return to play depends on range of motion and strength. Athletes should progress slowly and be able to perform movements that mimic their sport before attempting full participation.

MCL (Medial Collateral Ligament) Tear

Medial Collateral Ligament Tear is commonly seen in skiers as well as in football and soccer players, particularly in athletes 16-50 years old. The ligament tear or sprain can occur in a contact or non-contact with valgus force (force from outside being push inward).

The pain is typically localized and the athlete feels a tearing sensation or hears a popping sound. The most significant tenderness will be at the insertion of the upper knee.

For severe isolated MCL injuries, a period of bracing might be necessary to assist in ligament healing. For low grade MCL injuries, no bracing is required but ice massage, exercise bike as tolerated, and quadriceps strengthening exercises as well as a medial-lateral sleeve might provide additional support.

The initial goal is to regain full motion which may be stiff and painful in the beginning. As progress is made, the athlete should complete an exercise program for quadriceps, hamstrings and the muscles of the hip.

Recovery can take up to three months, depending on the range of motion, strength, and absence of pain. Athletes should progress slowly and gradually increase the time spent in their sport.

In this video below, I am working with one of my players who suffered a Grade 1 MCL tear. I am using the Vertimax to work on eccentric leg strength as part of our post-physical therapy rehab.
